What is the GUIDE Program?
Comprehensive dementia support
through GUIDE
The GUIDE (Guiding an Improved Dementia Experience) program, created by the Centers for Medicare & Medicaid Services (CMS), provides comprehensive support for individuals living with dementia and their caregivers. The program aims to:
Improve quality of life for dementia patients
Reduce caregiver stress
and burden
Help patients remain safely at home and active within their communities
Participants receive personalized care navigation, education, ongoing support, and respite care options through an interdisciplinary care team. Depending on eligibility, the GUIDE program could be available at no cost for qualified participants through Medicare. Eligibility criteria set by CMS may apply.
